This form is required for any player who has previously played representative basketball in one of the Waratah Junior Leagues (JPL, MJL, NEJL, SJL or WJL) who is wanting to trial for representative basketball with another association in one of the Waratah Junior Leagues (i.e. the last time you played representative basketball you did not play for the association you wish to trial for).

Note that this is NOT A CLEARANCE and players may not play in any Basketball NSW conducted competitions without a processed clearance.

Once this form is submitted all associations involved and Basketball NSW will receive an email notifying them of your application.
